Personal information:

We collect your Personal Information for the primary purpose of providing our services to our patients.  As well as communicating and providing information to our clients, for administrative purposes, billing for our services, to conduct internal audits and reviews, IT purposes, to obtain feedback about products and services to you, advertising, and marketing. For advertising and marketing- to send promotional information on our products and services.

When we collect Personal Information, we will, where appropriate and where possible, explain to you why we are collecting the information and how we plan to use it.

This Personal Information is obtained in many ways including interviews, correspondence, by telephone, in writing, our online booking system, Facetime, Skype, by email, via our website, from your website, from media and publications, from other publicly available sources, from cookies and from third parties.

This information may be stored on our computer medical records system, booking system and/or in handwritten medical records.

Wherever practicable we will only collect information from you personally. However, we may also need to collect information from other sources and third parties such as treating specialists, pathologists, other healthcare practitioners, your GP or hospitals.

  • Your contact details, including email address and/or telephone number,

  • Your demographic information, such as postcode,

  • Gender,

  • Health information,

  • Medical history and family history,

  • Debit card and credit card details for deposits and payments,

  • information you provide to us through our website, email, SMS, timely online appointment system, customer surveys, telephone, and social media.

  • Additional personal information that you provide to us, directly or indirectly, through your use of our website or booking system, from which you permit us to collect information.

  • Any other personal information requested by us and/or provided by you or a third party.

 

Consultation:

All patients need a consultation first prior to any treatment: to check their suitability, discuss and review their medical history, discuss the risks associated with the treatment, be given the opportunity to ask questions, gather more information about the treatment, discuss post care instructions, discuss cost of the treatment and also screen for BDD (Body Dysmorphic Disorder).

For all Schedule 4 medications patients will be given the opportunity to ask any questions with a medical practitioner and will also be reviewed by a medical practitioner prior to the treatment.

​

Restore My Glow engages multiple external third-party providers for the purposes of undertaking the consultation with a Health Practitioner, and approving the treatment.

​

Restore My Glow has the right to refuse treatment or terminate any service at any time without notice or reason.

​

You agree to disclose all your medical history.

​

That the information provided is true, accurate, not misleading and complete.

​

You consent to being 18 years old or more.

​

You agree to contact Restore My Glow if any concerns are raised regarding the treatment.

​

You agree to present in person for a review (if concerns are raised) so that a full examination can occur. This may or may not include the prescribing doctor who authorised the treatment.

​

You agree to follow the pre and post care instructions given to you.

 

Fees and Payment:

You agree to pay us the fee or fees for the service.

​

You acknowledge and agree that additional procedures or treatments may be required in order to achieve desired results. To the extent permitted by applicable law, any further treatment or services, including the treatment of any side effects or complications, will incur additional fees over and above the fees paid for the services.

 

These may include a referral to a specialist, where they may charge for their services.

​

You agree that reimbursement will not be provided on any treatment provided, unless a product is proven faulty.

​

You agree that we are not liable for any cost of any further treatments with other health care providers or any after care costs or expenses by another health care practitioner.

 

All prices listed on this site and in clinic are in Australian dollars and are inclusive of any Australian Goods and Services Tax (as applicable).

 

Gift Vouchers and Certificates:

Any defaced, altered, lost, or stolen gift vouchers will not be replaced, refunded or redeemed.

​

Purchases exceeding the available balance on the gift voucher, will need to be paid for with other payment options available in clinic.

​

Cannot be redeemed for cash and can only be used for treatments performed by us.

Must be used before the expiry date.

​

Cannot be returned for a refund, redeemed for cash, be replaced after expiry or have the balance transferred to a new gift voucher.

​

Expire 36 months after date of purchase and will be void and no longer redeemable after that date.

​

Restore My Glow takes no responsibility or liability for lost or damaged gift vouchers.

 

Packages and pre-paid treatment bundles:

To be paid upfront to receive the discount in the package.

​

Cannot be transferred to anyone else or any other clinic to receive the treatment.

​

Must be used at the clinic you purchased it from.

​

Are to be used within 12 months from the first treatment.  These packages are designed to be used in a series, and within a specific time frame, to achieve optimal results. E.g., skin needling, PRP, skin booster injections recommend 3 treatments spaced weeks apart to achieve optimal results.

​

At the conclusion of 12 months, and if all treatments have not been used, any remaining treatments will be forfeited and Restore My Glow takes no responsibility or liability for this, and is exempt from providing any of the treatments in the package.

 

Cancellation policy and re scheduling:
Restore My Glow values your time, as I hope you do ours. A 24-hour cancellation policy applies. 

​

Patients are required to give a minimum of 24 hours’ notice to cancel their appointment.  

​

Confirmation of your appointment is sent out via text message 48 hours before your appointment. This allows for re scheduling, if needed, via the online booking system.

​

In addition, confirmation emails are sent out 24 hours before your appointment.

​

Both the email and text confirmations confirm your upcoming appointment.

​

Failure to attend your appointment, or not giving sufficient 24 hours’ notice will incur a $50 cancellation fee.

​

After multiple no shows you will be blocked from booking any further appointments by the online booking system.

​

If you have bought a package, or a series of treatments, and fail to attend your appointment, or give insufficient notice of cancellation, you will forfeit one treatment as a cancellation fee.

 

Photography:

Prior to any treatment we may undertake photographs and videography of you.  You consent to this for the purposes of review and to monitor your treatment progress.

 

These may be shared with other health care practitioners from time to time, who are also involved in your care.  E.g., prescribing doctor or doctors.

Images will not be posted, submitted, or shared without prior consent from you. With your consent, they will be de-identified or identified. Intended use may be for marketing and advertising through social media, website, brochures, leaflets, posters, blogs, online, articles and educational resources. You can withdraw consent at any time.

​

You agree that you will not take personal recordings including photographs, audio and video of the clinic or anyone in it, or your consultation or treatment without our prior written consent.

 

Intellectual property:

We hold the copyright to the content of this web site.  You must not use or replicate any of the copyright material.

 

Unsubscribe:

You may unsubscribe from our mailing/marketing lists at any time by contacting us in writing via the contact details in this policy, or to our Privacy Team.

 

Sensitive Information:

Sensitive information is defined in the Privacy Act to include information or opinion about such things as an individual’s racial or ethnic origin, political opinions, membership of a political association, religious or philosophical beliefs, membership of a trade union or other professional body, criminal record or health information.

Sensitive information will be used by us only for the purpose for which it was obtained and with your consent, or where required or authorised by law.

 

Third Parties:

Where reasonable and practicable to do so, we will collect your Personal Information only from you. However, in some circumstances we may be provided with information by third parties. In such a case we will take reasonable steps to ensure that you are made aware of the information provided to us by the third party.

Third parties may include Google Analytics or other relevant businesses, other health care practitioners, the Timely online booking system and third-party banking to verify payment for deposits, gift vouchers or purchases, and making payments including After pay, ZIP pay and Square.

Our Site may contain links to other websites. We do not have any control over those websites, and we are not responsible for the protection and privacy of any personal information which you provide whilst visiting those websites. Those websites are not governed by this Privacy Policy.
